Case of the week:
A male in his 20s with Achondroplasia presents with headaches, transient visual obscurations, and pulse synchronous tinnitus.
He had disc edema grade 3 bilaterally.
He was found to have obstructive hydrocephalus secondary to a Chiari malformation.
He had an occipital craniectomy for relieving the obstruction.
MRI brain with a sagittal view below shows the cerebellum obstructing the 4th ventricle post occipital craniectomy.
The patient was sent for a VP shunt in addition to the craniectomy in order to alleviate his increased intracranial pressure secondary to obstructive hydrocephalus.
